Dermatology Billing: Codes and Denials We Manage
Dermatology claims mix medical and cosmetic services, procedure-heavy visits and pathology coordination, which makes modifier use and medical necessity documentation critical.
Codes and rules our dermatology billers work with:
- Destruction of premalignant lesions such as actinic keratosis (17000, 17003, 17004)
- Skin biopsies by technique (11102 to 11107) and benign lesion destruction (17110, 17111)
- Mohs micrographic surgery (17311 to 17315)
- Coordination with pathology for specimens (for example 88305) so biopsies and results link correctly
Common dermatology denials we prevent and appeal:
- Modifier 25 on an E/M performed with a procedure
- Cosmetic versus medically necessary treatment
- Lesion counts, and modifier 59 or X modifiers when procedures are distinct
- Biopsies not matched to pathology results
Practice Perfect handles dermatology charge entry, coding, claim submission, denial management and payer credentialing. Every new practice starts with a free billing audit that shows where revenue is leaking.

Dermatology Billing Questions Answered
When can an E/M visit be billed with a same day procedure?
Only when the visit is a separate, significant service reported with modifier 25. An E/M visit performed only to decide whether to do a minor procedure cannot be billed separately.
Does a new patient visit qualify for modifier 25 on its own?
No. The fact that the patient is new to the physician is not enough to report an E/M visit with a minor procedure.
Can a biopsy and a destruction be billed on the same lesion?
Not separately. NCCI edits bundle them when they are done on the same lesion in the same session. A biopsy of a different lesion can be reported separately with the right modifier.
Why does modifier 25 draw audits?
Dermatology uses it on a large share of E/M visits, so payers check that the note supports a truly separate service. We review documentation before the claim is sent.
What do your dermatology billing services cover?
We manage the complete billing cycle, including dermatology coding, claim submission, denial management and detailed reporting.
How do I start with a free audit?
Ask for a review on our free audit page. We go through your claims and point out the denials and delays costing you revenue.
