Oncology Billing Questions We Hear
How is chemotherapy infusion timed?
Code 96413 covers the initial hour of an infusion. Add on code 96415 is used for each additional hour and always needs 96413 on the same claim. An infusion of 15 minutes or less falls to the push code, 96409.
Why are infusion claims denied?
Common reasons are billing 96415 without 96413 and incomplete start and stop times in the nursing notes.
What are the JW and JZ modifiers?
For separately payable drugs from single dose vials, Medicare requires JW (discarded amount reported) or JZ (no waste) on the claim. Claims without one of them are returned as unprocessable. Multi dose vials are treated differently.
Can an E/M visit be billed on infusion day?
Only when it is separately identifiable from the pre infusion assessment that is bundled into the administration code, with modifier 25 on the E/M.
