At Practice Perfect, we specialize in delivering professional medical billing services in North Carolina for clinics, hospitals, and private practices of all sizes. Our team understands North Carolina-specific payer rules, Medicaid regulations, and commercial insurance processes, ensuring that every claim is clean, compliant, and paid faster.
Whether you’re running a small practice or a multi-specialty group, we tailor our solutions to your workflow so you can focus on patient care while we handle the billing complexity.
North Carolina Medical Billing: Payers, Medicare and Medicaid
Practices in North Carolina bill NC Medicaid Managed Care, Medicare Part B through Palmetto GBA (Jurisdiction M), and commercial plans such as Blue Cross and Blue Shield of North Carolina, alongside national carriers like Aetna, Cigna, UnitedHealthcare and Humana. Each payer has its own enrollment, authorization and claim-edit rules, so a claim that is clean for one payer is often rejected by another.
What we check for North Carolina practices before claims are submitted:
- Payer enrollment and credentialing for every provider and location, including Medicaid managed care plans
- Patient eligibility verification at scheduling, because coverage often moves between Medicaid, Medicare Advantage and commercial plans
- Prior authorization requirements by payer and service
- ICD-10, CPT and HCPCS coding and modifier accuracy against current payer policy
Practice Perfect provides outsourced medical billing, coding, credentialing and revenue cycle management for North Carolina practices. Every new practice starts with a free billing audit that shows where claims are being denied or delayed.
North Carolina Payers We Bill Every Day
North Carolina has one dominant statewide insurer, a Medicaid program that moved to managed care in recent years and a mix of large metro practices and rural eastern and mountain clinics. Our team bills all of the major payers:
- NC Medicaid: run by the North Carolina Department of Health and Human Services’ Division of Health Benefits. Most members are enrolled in Standard Plans run by health plans such as AmeriHealth Caritas, Carolina Complete Health, Healthy Blue, UnitedHealthcare Community Plan and WellCare, while members with significant behavioral health needs are served by Tailored Plans. Fee-for-service claims go through the state’s NCTracks system.
- Original Medicare: Part B claims processed by Palmetto GBA, the Medicare Administrative Contractor for North Carolina (Jurisdiction M, shared with South Carolina, Virginia and West Virginia).
- Commercial insurers: Blue Cross and Blue Shield of North Carolina, Aetna, Cigna, UnitedHealthcare, Humana and Ambetter.
- Medicare Advantage: plan choices change from year to year, so coverage should be verified at every visit.
- Other payers: TRICARE for military families near Fort Liberty, Camp Lejeune and Seymour Johnson Air Force Base, and workers’ compensation claims under the North Carolina Industrial Commission’s rules.
Each payer has its own portal, prior authorization list and appeal process. We track them so your claims go out clean the first time.
NC Medicaid Billing Rules Practices Should Know
NC Medicaid has several rules that catch practices off guard:
- NCTracks enrollment and plan credentialing: providers enroll with NC Medicaid through NCTracks, and each Standard Plan and Tailored Plan has its own credentialing and contracting on top of that. See our medical credentialing services.
- Claim handling timelines: health plans are expected to tell providers within 18 days whether a claim is clean, and to pay or deny clean claims within 30 days. If requested information is not sent in time, plans can deny the claim.
- Timely filing has changed: filing deadlines for health plans have been set at 180 days and later at 365 days, and fee-for-service claims through NCTracks have a 365-day limit. Check each plan’s current handbook, and track claim ages from day one.
- Eligibility changes: members can change plans and coverage can change month to month, so eligibility should be checked before every visit. See our insurance eligibility verification services, and our prior authorization services for plan-specific requirements.
North Carolina Prompt-Pay Rules for Commercial Claims
North Carolina’s prompt-pay law (G.S. 58-3-225) requires an insurer, within 30 calendar days of receiving a claim, to send the provider either payment, a denial notice, or a notice that proof of loss is inadequate or the claim was not on the required form. Late payments accrue interest at 18% a year starting the day after the claim should have been paid, and if the insurer asked for more information, interest starts on the 31st day after it receives that information. Many practices never collect that interest because nobody tracks claim ages against the deadline. Our A/R follow-up flags late-paid claims so you know when a payer is out of compliance.
Billing for Rural and Eastern North Carolina and the Research Triangle
North Carolina has large rural areas, especially in the east and the mountains, where practices rely on rural health clinics, federally qualified health centers and telehealth billing, which follow special payment rules. North Carolina expanded Medicaid in December 2023, so many more adult patients are now covered, which makes eligibility checks and correct plan billing especially important.
The state is also home to large health systems and a major research and biotech hub in the Research Triangle, with many group practices and academic practices. We support practices of every size, from solo providers to multi-location groups that need consistent credentialing, billing and reporting.
Medical Billing Across North Carolina
We support practices throughout the state, including Charlotte, Raleigh, Greensboro, Durham, Winston-Salem, Fayetteville, Cary, Wilmington, Asheville and Greenville, fully remotely.
Common North Carolina practice types we work with:
- Primary care and family medicine clinics serving Medicare, Medicaid and commercial patients.
- Behavioral health practices where Tailored Plans and Standard Plans have different rules.
- Specialty groups in cardiology, orthopedics and other specialties with heavy prior authorization requirements.
- Urgent care and pediatric practices with high volume and mixed payers. See our urgent care billing services.
