Family Practice Billing: Codes and Denials We Manage
Family practice claims are dominated by evaluation and management (E/M) coding, preventive care and chronic care programs, where the level billed has to match the documentation.
Codes and rules our family practice billers work with:
- Office visit E/M codes 99202 to 99205 (new patients) and 99212 to 99215 (established patients)
- Preventive visits (99381 to 99397) and Medicare Annual Wellness Visits (G0438, G0439)
- Chronic care management (99490) and transitional care management (99495, 99496)
- Modifier 25 when a procedure or vaccine is billed with a separate E/M
Common family practice denials we prevent and appeal:
- Modifier 25 missing or not supported by documentation
- Preventive and problem-oriented services billed together without correct coding
- An E/M level that the documentation does not support
- Vaccine product and administration codes not linked, or chronic care time thresholds not documented
Practice Perfect handles family practice charge entry, coding, claim submission, denial management and payer credentialing. Every new practice starts with a free billing audit that shows where revenue is leaking.
